We are a 143-bed facility offering rehabilitative services and long term care for residents with moderate to complex medical conditions, and a secured unit for Alzheimer’s care for residents with dementia.

We are part of Kissito Healthcare, a leading not-for-profit provider of innovative post acute rehabilitative and long term care healthcare services. Our growing family includes post acute facilities across the U.S.

Our Philosophy of Care

Our unique Collaborative Patient Care Pathway™ treats each resident as an individual. Interdisciplinary teams of physicians, nurse practitioners, licensed nurses and therapists work closely with residents and their families to develop personalized care plans based on their special needs, preferences and life experiences.

Rehabilitative Care

When patients are discharged from the hospital, our goal is to produce smoother care transitions and improved outcomes. We rigorously “track and trend” the progress of each resident to ensure the best care in a setting that includes:

  • Private and semi-private rooms
  • Rehabilitation gym
  • Physical, occupational and speech therapy
  • Vital-Stim for Stroke patients
  • Innovative art therapy and other programs

Long Term Care

For those who need a longer stay, we offer extended care services for medically complex and Alzheimer’s residents, including those with the following needs:

  • Tracheotomies
  • Dialysis
  • Wound Care
  • Dementia

Our Care Environment

We believe a warm, nurturing environment is essential to healing. Residents are encouraged to decorate their own rooms (fire retardant decorations required), and families and friends enjoy unrestricted visiting hours.

Activities are structured around individual needs and include innovative special programs to stimulate and engage each resident’s personal interests. Residents enjoy outings, movies and therapeutic interacting activities. Also, our registered dieticians make every effort to accommodate individual and cultural preferences.
