Our unique Collaborative Patient Care Pathway™ treats each resident and patient as an individual. Interdisciplinary teams of caregivers — physicians, nurse practitioners, licensed nurses and therapists — work closely with residents/patients and their families to develop personalized care plans based on their needs, preferences and life experiences.
Therapists work closely with medical staff and monitor patient progress daily. Physicians and nurse practitioners are present on a regular basis to assist with any changes in residents’ condition.
